All students must be up-to-date with physical examination and immunization requirements and all completed forms must be turned into the School Health Office before August 18, 2021.
Students in 9th grade must have a physical examination and all required immunizations.
Students in 12th grade must show proof of receiving two doses of meningococcal vaccine (meningitis) with the second dose received on or after the 16th birthday. If the first dose was given after the age of 16, then only one dose is required.
