Students will NOT be able to attend school after October 16, 2023 until ALL required information is received.

Requirements by Grade Level

Ninth Grade Students:

• Physical Exam (Sports physical is NOT acceptable)

• Complete immunization record signed by medical provider

• Dental Exam on Illinois form due by May 15, 2024 (dated within 18 months of the May 15th deadline)

Twelfth Grade Students:

• Proof of receiving (2) Meningococcal vaccines:  First dose received on or after the 11th birthday and second dose received on or after the 16th birthday.  (Only one dose is required if the first dose was received at 16 years of age or older.)

Thursday, October 16, 2024 is the deadline for all required health information.
